Job Description
Join The Jewish Board as a Case Manager to empower individuals with serious mental illness to achieve independence in their community. Use your skills to support residents in recovery, advocate for their needs, and create personalized service plans in a compassionate environment.
Key Responsibilities
Carries a caseload and practices person-centered and collaborative service planning
Evaluates and assesses independent living skills
Advocates for residents with other service providers
Completes regular documentation of resident interactions
Participates in intake, assessments, discharge, and follow-up conferences
Facilitates support groups and resident meetings
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
