They gather and analyze clinical data including physical assessments, treatment plans, lab results, and medication lists, ensuring timely and accurate hospice eligibility evaluations. The role requires advanced nursing qualifications, state licensure, and national certification, focusing on compassionate care delivery in home-based hospice settings.
Overview:We’re looking for a Hospice Nurse Practitioner to join our team. You will report directly to the Executive Director or Administrator. You will be assigned to a location to perform Face-to-Face (F2F) encounters and be responsible for gathering clinical information to support the hospice diagnosis and prognosis. These findings are then communicated to the hospice physician responsible for certifying or recertifying the patient's hospice eligibility.
The scope of practice for the hospice NP is limited to those tasks necessary to complete hospice F2F encounter visits.
