ProKatchers is seeking a travel nurse RN Hospice for a travel nursing job in Sunnyvale, California.
POSITION SUMMARY:
This role coordinates care with a multidisciplinary team, participates in the hospice program's quality assessment performance improvement program and conducts follow up visits as needed.
The Hospice Nurse Case Manager admits patients to service, assesses the patient’s and family’s physical, psychosocial, environmental, safety and developmental needs.
They develop an individualized plan of care based on their assessment and document their observations and interventions related to the patient's environment.
1 Year of Hospice Nursing Experience Required
Territory Details: Atherton, Menlo Park, Palo Alto, Mountain View, Sunnyvale, Santa Clara, San Jose, Saratoga, Los Gatos, Campbell, Morgan Hill, Gilroy and neighboring cities as needed.
