Join Epitec as a Member Care Coordinator, where you'll provide essential telephonic support to members by conducting health assessments and guiding them through their care plans. This role emphasizes compassionate communication and collaboration with clinical teams to enhance member health outcomes.
Key Responsibilities
Conduct home health assessments and system updates
Perform outreach and follow-ups with members on their care plans
Build relationships to ensure care plan compliance
Educate members and update records
Check in with members on care plan goals
Meet production targets based on department needs
Support clinical team with non-clinical tasks
Handle member and provider inquiries
Report system issues to help desk
Update customer/provider data
Gather missing information via calls or correspondence
Liaise on claims and refer cases to clinicians
Coordinate with internal departments on concerns
Communicate professionally with all co-workers, management, and customers
Follow HIPAA, diversity, and compliance policies
Maintain confidentiality
Report updates to management and assist with special projects
Required Qualifications
Bachelor of Social Work or Psychology
LVN/LPN with 1 year experience in managed care systems
RN with 3 years of care coordination for a state managed or waiver program
