Key Responsibilities
Provide direct clinical services in the Collaborative Care Model (CoCM)
Coordinate care with medical providers and other mental health providers
Screen and assess patients for mental health and substance abuse disorders
Monitor patients for changes in clinical symptoms and treatment side effects
Support psychotropic medication management and treatment adherence
Provide brief behavioral interventions using evidence-based techniques
Participate in caseload consultation with psychiatric consultants
Required Qualifications
Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), Licensed Advanced Practice Social Worker (LAPSW), Licensed Mental Health Counselor (LMHC), Licensed Professional Counselor - MHSP (LPC-MHSP), Licensed Professional Counselor - MHSP-Temporary (LPC-MHSP/T), Licensed Marriage and Family Therapist (LMFT), or Licensed Marital and Family Therapist - Temporary (LMFT/T)
3+ years of experience treating patients with behavioral health conditions
1+ years of experience working in primary care or integrated medical settings
Experience with assessment and evidence-based psychosocial treatments for mental health disorders
Preferred Qualifications
Experience in the Collaborative Care Model (CoCM)
Detail-oriented and organized with exceptional follow-up capabilities
Ability to maintain effective relationships with patients and care team members
Experience working with patients in person, by telephone, and video
Solid grasp of technology solutions and tools
