Job Description

Key Responsibilities
Perform skilled nursing visits and coordinate client care
Act as point of contact for all disciplines involved in patient care
Oversee frequency of visits for the episode
Consult with the physician and office regarding patient care details
Perform accurate OASIS collection and maintain current medication profile
Ensure timely reporting of lab values to the physician
Attend weekly case conference and monthly case manager meetings
Required Qualifications
Currently licensed as a Registered Nurse (RN) in the state of employment
Minimum of one year of clinical experience
Knowledge and skill in current nursing practice
Valid state driver’s license and automobile liability insurance
