Job Description
Join The Jewish Board as a Case Associate and make a significant impact in the lives of New Yorkers. You'll be responsible for developing client care plans, conducting community outreach, and ensuring access to essential services.
Key Responsibilities
Develop action plans for client care
Conduct routine community contacts on behalf of social workers
Locate and utilize community resources for clients
Evaluate living conditions of clients through home visits
Provide structure and guidance for independent living skills
Run groups under the supervision of social workers
Perform individual/group counseling based on client needs
Provide case management to clients
Establish community linkages for client support
Complete necessary documentation and progress notes
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant in a community or social service group
Preferred Qualifications
Bilingual Haitian-Creole
