Physical Therapist

Key Responsibilities:

• Perform comprehensive evaluations to assess residents’ mobility, strength, and therapy needs.
• Develop and implement personalized treatment plans to improve residents’ functional abilities and quality of life.
• Provide therapeutic interventions, including exercises and modalities, to help residents achieve their goals.
• Monitor progress, adjust treatment plans as needed, and provide education to residents and their families.
• Collaborate with an interdisciplinary team to deliver exceptional patient-centered care.
• Document assessments, treatments, and progress accurately and in compliance with facility and regulatory guidelines.

Qualifications:

• Current Physical Therapist (PT) license in New York.
• Graduate of an accredited Physical Therapy program.
• Experience in a Skilled Nursing Facility (preferred but not required).
• Strong communication and interpersonal skills.
