- Monday - Friday, 8am - 5pm schedule with hybrid work model
- 12 - 15 patients per day
- Clinic-based setting with home health visits
- Split schedule: 3 days field work, 2 days telehealth
- Geriatric patient population focus
- Hybrid role combining telehealth and home health visits
- Three years adult primary care experience required
- Must be comfortable working independently
