Physical Therapist (Home Health)

1. Performs clinical assessment to identify patient needs and appropriateness of patient. 
2. Document, prepare/review and complete a Plan of Care (POC), progress notes, phone orders and other clinical record documentation on an ongoing basis. Revise the POC based on evaluation, ongoing assessment data. Works in consultation with the physician and as part of an interdisciplinary team.
3. Plan and provide appropriate treatment for neuromuscular, neurological and orthopedic disorders based on medical referral and therapy assessments.

Current State License as a Physical Therapist - Required Proof of current CPR certification - Required Current driver's license. Must have reliable transportation and insurance - Required 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required Previous home care experience - Preferred
