Job Description

Join Mount Carmel as a Medication Historian, where you'll play a vital role in compiling accurate patient medication histories. You'll interact with patients and their families, ensuring safety and quality in medication management while benefiting from a supportive work environment and competitive compensation.

Key Responsibilities

  • Gather information about the patient's medications from family or directly from the patient
  • Compile a complete list of patient's medications
  • Collect information on patient's drug allergies and reactions
  • Enter obtained information into appropriate application/system
  • Communicate with physician offices, retail pharmacies, or other sources to verify medications
  • Compare medication list to indications and medical history
  • Identify medication discrepancies and facilitate correction or escalation
  • Demonstrate responsibility and act independently

Required Qualifications

  • High school graduation or equivalent
  • Approved Pharmacy Technician Certification
  • Active registration as a Certified Tech with Ohio State Board of Pharmacy or active Pharmacy Intern License
  • 1-3 years of outpatient pharmacy experience preferred
  • Familiarity with drug names and indications
  • Ability to interact efficiently with patients and families
