Key Responsibilities
Provide social casework to individuals and families receiving services
Assess family's financial situation and refer for financial assistance
Respond to referrals for casework by Agency staff or outside professionals
Participate in Agency IDG team conferences to identify social problems
Refer patients and families to community agencies with follow-up
Participate in case conferences with other agencies
Interpret social resources to staff and health services
Ensure the use of the 4Ms and provide Age-Friendly Care
Required Qualifications
Master's degree (MSW) from an accredited school of Social Work
Current Social Worker License in the state of practice
Valid driver’s license and good driving record
Proficiency in clinical skills
Ability to make sound professional clinical judgment
Ability to assess and document patient needs
Excellent verbal and written communication skills
Proficiency in personal computer use
Preferred Qualifications
Advanced Palliative Hospice Social Worker Certification-APHSW-C
