**Key Responsibilities:**
• Provide physical therapy in a home health setting.
• Complete Homecare Homebase documentation on•site.
• Attend weekly in•person office meetings.
• Offer flexible scheduling to meet patient needs.

**Requirements:**
• Valid PT License
• BLS Certification
• 2+ years of recent Physical Therapist experience
• Recent home health experience with HCHB/OASIS
• Reside within 30 minutes of the territory or secure nearby housing

**Contract Details:**
• 13•week contract, 40 hours/week
• No on•call or overtime
• Mileage reimbursement

**Additional Information:**
• 2•3 days of orientation
