Travel Nurse RN - Case Management - $1,940 per week

Key Responsibilities

• Perform utilization reviews to ensure medical necessity of admissions and continued stays
• Develop and execute discharge planning in collaboration with the care team
• Manage financial risk and payor communications
• Collaborate with the interdisciplinary care team
• Complete all medical record documentation in a timely and accurate manner
• Coordinate community resources and provide patient/family education
• Identify barriers to care and create solutions
• Ensure adherence to MedHost UR system documentation and reporting protocols
• Serve as a facility representative in communications with payors and outside providers
• Provide coverage for Director of Case Management as needed
• Track and submit financial projections and critical billing forms
• Maintain up-to-date and accurate case records
• Provide weekly commercial projections to Corporate Finance
• Ensure compliance with national and local contract obligations
• Participate in outlier management and quality improvement meetings
