Key Responsibilities
Carry a caseload of residents and provide services within program regulations
Complete client assessments including intake and risk assessments
Evaluate and support clients in meeting individual goals
Practice person-centered service planning with clients
Develop safety plans and provide individual and family counseling
Make referrals and coordinate services for clients
Advocate for residents with other service providers
Participate in weekly case conferences and regular staff meetings
Complete documentation of all resident interactions
Required Qualifications
Master's degree in Social Work
LMSW or LCSW certification preferred
A minimum of 1-year experience in Human Services preferred
Preferred Qualifications
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
