Required Qualifications:

• Home Health experience - McKesson/Netsmart EMR experience preferred
• Collaborate with interdisciplinary team and colleagues to ensure a person’s needs are met.
• Provide occupational therapy care to a broad cross-section of clients living in central Vermont.
• Offer education and support to individuals to help them set and reach their goals of care.
• Support your clients’ desire to maintain their independence and to live in their own home.

License / Certification:

• Vermont OT licensure
• General Certifications(ACLS,BLS/BCLS,PALS,NRP);
• Rehabilitation Certifications(ASHA,CHT,ACP Training )

Shift Notes:
M-F w/ weekend rotation
