Job Description
Join The Jewish Board as a Case Associate and make a significant impact by supporting clients in navigating community resources and developing personalized care plans. You'll work closely with clients and their families to ensure they receive the necessary services and guidance for their well-being.
Key Responsibilities
Function as a case planner supplying appropriate interventions for client care
Develop action plans utilizing existing and new resources for clients
Conduct routine community contacts and home visits
Evaluate clients for individualized treatment programs
Perform individual/group counseling
Provide case management and community outreach
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant
Preferred Qualifications
Bilingual Haitian-Creole
