Key Responsibilities
Manage a caseload of members
Develop and modify Individual Care Plans
Provide psychosocial support and address health-related social needs
Ensure members receive services in the least restrictive setting
Collaborate with Community Health Workers and other agencies
Monitor patient care and progress towards outcomes
Employ various strategies to manage health issues
Required Qualifications
Must reside in Virginia (specific counties listed)
Bachelor's degree in health or human services or active LPN license
Two years of prior experience in health care and/or case management
One year of experience with individuals meeting Cardinal Care Priority Population criteria
Strong advocacy for members
Intermediate to advanced computer skills
Exceptional communication skills
Ability to manage multiple priorities
Patient-facing role requiring TB screening
Valid state driver's license and personal vehicle insurance
Preferred Qualifications
Experience with Medicare, Medicaid, and dual eligible populations
Experience in health promotion, coaching, and wellness
Knowledge of community health and social service agencies
Case Management Certification (CCM)
Bilingual or Multilingual capabilities
