Key Responsibilities
Provides direct patient care as defined in the state Nurse Practice Act
Implements plan of care initiated by the Registered Nurse
Provides accurate and timely documentation
Assesses and provides patient and family education
Participates in the coordination of home health services
Follows infection control standard precautions
Communicates with the physician regarding patient needs
Participates in on-call duties as assigned
Required Qualifications
High school diploma or equivalent
Graduate of an accredited practical nurse or vocational nursing program
Current unrestricted license in Texas as a Licensed Vocational Nurse
Current CPR certification
2+ years clinical experience in community home health or medical/surgical setting
Thorough knowledge of current nursing practice and regulations regarding home care
Valid Texas Driver's License and personal auto insurance coverage
Tuberculosis screening and proof of immunity to certain diseases
Preferred Qualifications
Experience working with electronic medical record applications
Basic computer skills including Microsoft Word and Outlook
