Key Responsibilities
Interviewing, education, training, and performance management of clinical team members
Client triage including admissions, recertifications, and discharges
Creating individualized Plans of Care with the care team
Reviewing documentation submissions for accuracy
Conducting home and/or hospital visits
Required Qualifications
Active unrestricted Registered Nurse (RN) license
At least one year of experience as an RN within the last 36 months
Ability to prioritize and handle multiple tasks
Familiarity with electronic charting
Excellent verbal and written communication skills
Preferred Qualifications
Pediatric Nursing (PDN) experience
