Job Description

Join CenterWell Home Health as an Occupational Therapist and play a crucial role in helping patients regain their independence and improve their quality of life through personalized home care. You'll work collaboratively with healthcare teams to develop and implement therapeutic programs that empower patients to overcome physical limitations.

Key Responsibilities

Assess/screen patient's daily living/work-related skills
Develop therapeutic retraining programs
Administer and interpret diagnostic tests of function
Confer with the patient's physician and health care team
Provide therapeutic treatment and instruction to patients
Design community reintegration activities
Coordinate/oversee/supervise Occupational Therapy Assistant performance
Document patients' care observations and interventions

Required Qualifications

Degree from an accredited Occupational Therapy Program
Current and unrestricted OT licensure
Current CPR certification
Good organizational and communication skills
Valid driver’s license and auto insurance

Preferred Qualifications

A minimum of six months of occupational therapy experience
Home Health experience
