TENTATIVE backfill - Assignment for 16-32 weeks

Hospice RN: Call coverage rarely needed but could cover on call team. Travelers must work 2 out of the 4 holidays: Thanksgiving, Christmas, Christmas Eve, and New Years Day.

Requirements:
• AHA BLS required
• A minimum of 3 years of prior community hospice experience
• Will need excellent typing skills as they have a narrative heavy EMR
• Hospice/End of Life-Complex symptom management and education
• Symptom assessment and management with high level of critical thinking skills
• Comfort initiating and participating in goals of care discussions and planning with patients, family, and team
• Medication management and administration (IV, Subcutaneous, oral, sublingual, rectal, and other routes)

• Monday to Sunday
• Bi-weekly Orientations on Monday , next orientation is 8/18
• No block schedules for any reason

At MMRH day nurses care for 5-7 patients (all levels of care) and overnights shift up to 11. Respiratory available 24/7 for support ☐ Y ☒ N Anesthesia available 24/7 for support ☐ Y ☒ N Who manages ventilators: N/A Unit Specific Procedures and Protocols: Pain management, starting pain continuous infusion, subcutaneous site insertion and maintenance central line care including PORT/IVAD, PICC, and other central lines, Palliative sedation, tracheostomy management and care, rectal tube management, wound care protocol, ostomy and other drain management, venipuncture for labs/specimens. Additional Unit Specific Information: End of life care assessment and care planning requiring strong comprehensive assessment skills. Symptom assessment and management with high level of critical thinking skills. Comfort initiating and participating in goals of care discussions and planning with patients, family, and team. Medication management and administration (IV, Subcutaneous, oral, sub-lingual, rectal, and other routes. Strong communication skills and ability to teach patients and caregivers aspects of end-of-life care. Generally, a traveler case manager is assigned a geographic area, not full territory. When they cover a weekend rotation-they would be expected to make visits throughout our service area- Chittenden & Grand Isle Countries. We have had travelers that float. Manager will post/express a need and be clear about expectations if outside of our norm. Strict protocol in place with following guidelines with use of app to call patients/families.
