Key Responsibilities
Independently perform home visits
Conduct functional, psychosocial and environmental safety assessments
Determine eligibility of enrollees
Establish individual plans of care
Required Qualifications
Current NYS RN license and registration
Associates degree
Bilingual BSN new graduates encouraged to apply
Preferred Qualifications
Bachelor of Nursing degree
One year of experience in Homecare, acute care or community health nursing
Working knowledge of Medicare, Medicaid and NYS DOH standards and regulations
