Job Description

Join UnitedHealthcare as a Field Care Coordinator, where you'll play a critical role in enhancing healthcare experiences for individuals in Southeast Wayne County, MI. This position involves developing personalized care plans and ensuring effective service delivery while working closely with patients and healthcare teams.

Key Responsibilities

  • Develop and implement care plan interventions
  • Communicate health-related information to stakeholders
  • Advocate for patients and families
  • Assess, plan, and implement individualized care strategies
  • Identify barriers to care and manage interventions
  • Initiate referrals for social service programs
  • Provide resource support to members
  • Manage person-centered service/support plans
  • Conduct home and in-person visits
  • Document member information and care management activities

Required Qualifications

  • Current, unrestricted independent licensure as a Registered Nurse (RN) in Michigan
  • Master’s degree and current, unrestricted independent licensure as a Social Worker (e.g., LMSW, LCSW, LLMSW)
  • Bachelor’s degree and current, unrestricted independent licensure as a Social Worker (e.g. LLBSW, LBSW)
  • 2+ years of experience in community health setting
  • 1+ years of experience with local behavioral health providers
  • 1+ years of experience working with long-term care needs
  • 1+ years of experience with electronic documentation systems and MS Office
  • Access to reliable transportation
  • Ability to travel within assigned territory
  • Access to a designated quiet workspace
  • Ability to travel to Southfield, MI for meetings
  • Must reside within Michigan

Preferred Qualifications

  • RN or LMSW; LCSW, LLMSW
  • 1+ years of medical case management experience
  • Experience in Motivational Interviewing, Trauma-Informed Care
  • Experience with co-occurring disorders
  • Experience with MI Health Link
  • Experience working in Managed Care
  • Knowledge of NCQA documentation standards
