Key Responsibilities
Assess, plan, implement, and coordinate care management activities
Develop personalized care plans and educate members and caregivers
Coordinate between members and care provider teams
Monitor progress towards care plan goals
Provide psychosocial and resource support
Identify problems and barriers for care coordination
Collect and maintain member information for compliance
Perform on-site visits to assess member needs
Serve as a Subject Matter Expert (SME) and preceptor for new staff
Required Qualifications
Bachelor's degree
4 - 6 years of related experience
Graduate from an Accredited School of Nursing (if holding clinical licensure)
Clinical licensure (if applicable)
Preferred Qualifications
Licensed practical nurse with a minimum of four years of pediatric experience
Master’s degree in social work with a minimum of one year of related pediatric care experience
