Join Aveanna Healthcare as a Full-Time Home Health Physical Therapist, where you will help patients recover in the comfort of their homes. Your role involves conducting assessments, creating care plans, and coordinating with healthcare teams to ensure optimal patient outcomes.
Key Responsibilities
Perform initial and ongoing assessments to evaluate mobility, coordination, strength and current level of function.
Administer, evaluate, and interpret tests and measurements to determine functional goals and outcomes.
Provide written evaluation and plan of care defining physical therapy needs, goals, progress and expected outcomes.
Deliver physical therapy services per prescribed physician orders.
Maintain plan of care through timely documentation and modification of goals.
Responsible for discharge planning and educating patients and their caretakers.
Coordinate services with healthcare team members.
Supervise other personnel (PTA, HHA) as applicable.
Complete OASIS assessments where allowed by state regulations.
Required Qualifications
Degree in Physical Therapy from an accredited university/program.
Current, unrestricted state license as a Physical Therapist in the state of practice.
Valid CPR
