Key Responsibilities
Perform initial comprehensive assessments on newly enrolled patients
Develop and update patient management care plans
Consult with patients and legal representatives regarding care plans
Perform basic nursing procedures including IV placement and wound care
Prescribe diagnostics and interventions to avoid unnecessary admissions
Complete follow-up and post-discharge assessments
Educate patients on disease processes and wellness management
Participate in meetings related to patient care and clinical excellence
Implement quality initiatives to ensure high standards of care
Stay current on medical and nursing research and technology
Required Qualifications
Completion of Nurse Practitioner or Physician Assistant accredited program
Current state RN/NP or PA license
Certification through AANP, ANCC, or NCCPA
Preferred Qualifications
2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
Knowledge of long-term care, community resources, and patient care delivery systems
Knowledge of CPT, ICD-10, and HCPC codes
Knowledge of clinical standards of care
Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts
