Key Responsibilities
Manage daily operations for the Care Coordination department
Develop and implement departmental goals and objectives
Assess quality of patient care and coordinate services
Collaborate on staff orientation and ongoing education
Monitor departmental budgets and regulatory compliance
Manage staff relations including performance management
Required Qualifications
Bachelor’s degree in nursing (BSN) or Master’s in social work (MSW)
Registered Professional Nurse (RN) or Licensed Clinical Social Worker (LCSW) certification
Preferred Qualifications
2 years experience as a manager or case manager
5 years experience with 2 years in progressive leadership
