Key Responsibilities
Function as a case planner supplying appropriate interventions for client care
Develop action plans and utilize existing resources for client care
Conduct routine community contacts and home visits
Evaluate living conditions and client needs
Provide individual/group counseling and case management
Perform community outreach and establish linkages with supportive groups
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant
Preferred Qualifications
Bilingual Haitian-Creole
