Key Responsibilities
Deliver cost-effective, quality care to assigned members
Manage both medical and behavioral, chronic and acute conditions
Perform comprehensive assessments and document findings
Ensure accurate ICD10 coding and documentation
Conduct medication reviews and reconciliations
Facilitate implementation of the member’s plan of care
Coordinate care across different levels and settings
Monitor and adjust the plan of care as needed
Communicate effectively with internal and external parties
Promote the CCM program in assigned facilities
Required Qualifications
Certified Nurse Practitioner or Physician Assistant
Graduate of an accredited master’s degree in Nursing (for NPs) or Physician Assistant degree program (for PAs)
Active and unrestricted license in the state
Current active DEA licensure/prescriptive authority or ability to obtain
Access to reliable transportation
Ability to lift a 30-pound bag and navigate various conditions
Availability to work 24 hours per month with off-hour expectations
Ability to gain a collaborative practice agreement if applicable
Preferred Qualifications
1+ years of hands-on post grad experience within Long Term Care
Understanding of Geriatrics and Chronic Illness
Understanding of Advanced Illness and end of life discussions
Proficient computer skills including electronic medical records
Ability to develop and maintain positive customer relationships
Adaptability to change
