Physical Therapist (PT) – Skilled Nursing Facility (SNF)

MedAdventures is seeking a skilled and compassionate Physical Therapist (PT) to join a well-established Skilled Nursing Facility (SNF) team in Altoona, FL. This is an excellent opportunity for a PT who is passionate about helping residents improve independence, mobility, and overall quality of life through evidence-based, functional therapy.

In this role, you’ll collaborate with an interdisciplinary care team to evaluate, treat, and support patients with diverse rehabilitation needs in a long-term care setting.

Key Responsibilities:

• Evaluate residents and develop individualized treatment plans focused on mobility, strength, balance, and function
• Provide skilled physical therapy services per care plans and physician orders
• Collaborate with OTs, SLPs, nursing staff, and families to promote optimal outcomes
• Educate patients and caregivers on home exercise programs, assistive devices, and safe mobility techniques
• Accurately document services provided, progress, and outcomes

Qualifications:

• Active Florida PT license (or eligibility to obtain)
• Experience in a skilled nursing or geriatric setting preferred
• Strong communication, clinical reasoning, and documentation skills
• Passion for patient-centered care and functional rehabilitation
