Key Responsibilities
Facilitates patient identification, screening, and enrollment for program eligibility
Monitors, manages, and addresses clinical pools and documents events in patients’ digital health record
Supports vital sign alerts
Collaborates with post-acute networks and community organizations
Facilitates scheduling of imaging and laboratory testing
Helps support care coordination and patient navigation
Performs prior authorizations and medication refill requests
Communicates with patients on test results
Reinforces providers' post-visit instructions
Provides patient and caregiver education
Maintains privacy and confidentiality
Attends in-services for professional development
Required Qualifications
0-2 years of experience
BLS Certification
Certified or Registered Medical Assistant
Preferred Qualifications
