Key Responsibilities

  • Facilitates patient identification, screening, and enrollment for program eligibility
  • Monitors, manages, and addresses clinical pools and documents events in patients’ digital health record
  • Supports vital sign alerts
  • Collaborates with post-acute networks and community organizations
  • Facilitates scheduling of imaging and laboratory testing
  • Helps support care coordination and patient navigation
  • Performs prior authorizations and medication refill requests
  • Communicates with patients on test results
  • Reinforces providers' post-visit instructions
  • Provides patient and caregiver education
  • Maintains privacy and confidentiality
  • Attends in-services for professional development

Required Qualifications

  • 0-2 years of experience
  • BLS Certification
  • Certified or Registered Medical Assistant

Preferred Qualifications

  • 3-5 years of experience
  • MA School or Associate Degree
