Job Description
Join The Jewish Board as a Case Associate, making a significant impact in the lives of clients through community-based planning and support. Develop action plans, provide case management, and utilize resources to ensure effective client care.
Key Responsibilities
Function as a case planner and supply appropriate interventions
Develop action plans to identify and utilize resources for client care
Conduct routine community contacts on behalf of social worker
Evaluate living conditions of clients through home visits
Provide case management and individual/group counseling
Perform community outreach and establish linkages with other organizations
Required Qualifications
BA Degree
Experience in a community or social service group or five years as a Case Assistant
Preferred Qualifications
Bilingual Haitian-Creole
