MedAdventures is seeking a travel Home Health Physical Therapist for a travel job in Mifflin, Pennsylvania. Job Description & Requirements

Responsibilities:

• Evaluate, develop, and implement individualized treatment plans for patients in a home health setting.
• Complete OASIS assessments and ensure compliance with home health standards.
• Provide therapeutic interventions to restore function, improve mobility, and reduce pain.
• Collaborate with referring physicians, healthcare professionals, and team members to optimize patient care.
• Educate patients and families on exercise programs, injury prevention, and post-treatment care.
• Maintain accurate and timely documentation of patient progress.

Qualifications:

• Active Physical Therapist (PT) license in PA.

• Ability to work independently and manage a flexible schedule.
