Job Description
Join The Jewish Board as a Case Manager, where you'll support adults with serious mental illness in achieving independence and stability. You'll work closely with residents, guiding them in their recovery journey while advocating for their needs in a compassionate, non-judgmental environment.
Key Responsibilities
Carries a caseload and practices person-centered and collaborative service planning with residents
Ensures service plans are completed and signed by residents according to regulatory guidelines
Supports residents in meeting individual goals and medication management independence
Advocates on behalf of residents with other service providers
Completes regular and timely documentation of all resident interactions
Participates in intake, assessments, discharge, and follow-up conferences
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
Preferred Qualifications
At least 1-year experience in Human Services
Familiarity with Microsoft Office suite of products
