Join Professional Case Management as a Senior Case Manager RN, where you can create your own schedule and enjoy a great work/life balance. You'll provide in-home care to former Nuclear Weapons Workers while leading a team of RN Case Managers to deliver high-quality care.
Key Responsibilities
Direct care and case management of assigned clients
Assist and collaborate with the regional director and other personnel to identify and correct issues and/or improve services
Plan, implement, and evaluate care provided
Coordinate and manage client care conferences
Serve as a local on-site clinical resource
Collaborate with staffing coordinators regarding appropriateness of staffing and scheduling
Ensure adherence to Agency policies
Participate in interviewing, selection, and ongoing evaluation of clinical personnel
Review and evaluate clinical documentation for accuracy and completeness
Participate in performance improvement initiatives
Required Qualifications
Graduate of a state approved school of professional registered nursing
Current, unrestricted RN license in the state(s) of practice
Minimum of two (2) years nursing experience including one (1) year in home care or closely related field
Current CPR certification
Preferred Qualifications
BSN preferred
One (1) year of supervisory and/or case management experience preferred
