MedAdventures is seeking a skilled and compassionate Physical Therapist (PT) to join our Home Health team serving Macedonia, OH and surrounding communities . This is a rewarding opportunity to provide essential therapy services in patients’ homes—helping them regain mobility, function, and independence while remaining in a familiar and comfortable environment.

Responsibilities:

• Provide physical therapy services to home-bound patients in accordance with individualized care plans
• Complete comprehensive evaluations and develop goal-driven treatment plans
• Document visits and progress notes accurately and in compliance with OASIS and Medicare guidelines
• Collaborate with nursing, case management, and other disciplines to ensure coordinated care
• Educate patients and caregivers on exercises, safety strategies, and treatment goals
• Manage a caseload efficiently within the assigned territory

Qualifications:

• Active Ohio PT license (or eligibility to obtain)
• OASIS experience required
• Prior home health experience strongly preferred
• Strong clinical reasoning, communication, and time-management skills
• Reliable transportation, valid driver’s license, and proof of auto insurance
