We are seeking a part-time Licensed Nurse to participate in the case management and direct care of residents to assure excellence in the delivery of health services including nursing care, rehabilitation therapies, social work, medications, activity programs and household duties.

Hours: Average of 32 hours
Responsibilities: Contribute to and maintain high quality Nursing Center services that meet and exceed staff and Resident expectations. Maintain commitment to excellence in carrying out nursing responsibilities. Promote a resident directed approach to care planning and problem solving. Participate in and assist the Director in the day-to-day functions of the unit as assigned. Complete rounds on unit each shift to assess resident needs, cleanliness, organization, and safety of home environment. Ensure meal service, housekeeping and laundry services, and resident transport to appointments and programs occurs as scheduled. Assist as needed. Write progress notes, enter orders, identify advanced directives, and print paperwork for appointments and other reports as appropriate utilizing EMR. Complete admission orders, nursing history, and assessments. Use only authorized abbreviations when recording information. Modify C.N.A. assignments. Retrieve messages and reset passwords in EMR. Coordinate nursing services with interdisciplinary team and consultants to ensure continuity of resident care. Ensure all nursing staff complete assigned responsibilities on the unit. Ensure department staff follow the established policies and procedures at all times. Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the department and facility. Review and revise resident care plans for appropriate goals, problems and approaches based on care needs; encourage resident participation. Ensure all staff follow the care plan when providing daily care to the residents as appropriate. Ensure all CNA documentation is complete by the end of the shift. Maintain privacy and confidentiality of all pertinent resident care information to ensure the protection of resident rights. Monitor nursing care to ensure all residents are treated with kindness, dignity, and respect. Observe and evaluate residents’ physical and emotional status at start and end of shift at minimum. Assist in finding coverage for call outs. Keep the Director informed of the status of residents, potential problems, and incidents, and assist in developing and implementing corrective action. Problem solve with residents, staff, families and visitor. Seek Director support as needed. Consult with resident's physician on resident care, treatment, and rehabilitation as necessary. Make independent decisions concerning nursing care. Other related duties as assigned. Assisted Care Only Oversee and ensure Aide medication cueing. Administer and monitor exception medications & treatments. Respond to emergency calls in residential units as assigned. Skilled Nursing Only Ensure medication and treatment schedules are administered as ordered. Assist with QAPI initiatives as assigned. Qualifications: Hold current nursing license in Commonwealth of Massachusetts (LPN or RN) Hold current CPR certification. Experience working in a long-term-care setting.
