Job Description
The Case Manager at The Jewish Board plays a crucial role in supporting adults with serious mental illness to live independently in the community. Utilizing a non-judgmental, harm-reduction approach, you will guide residents in meeting personalized recovery goals and develop essential life skills.
Key Responsibilities
Carries a complex caseload and practices person-centered service planning
Ensures service plans are completed and signed by residents
Supports residents in medication management independence
Advocates on behalf of residents with service providers
Completes regular documentation of resident interactions
Participates in intake, assessments, discharge, and follow-up conferences
Required Qualifications
Bachelor of Arts (BA) or Bachelor of Science (BS) degree
Experience working with individuals living with serious mental illness, substance use disorders, or trauma
Preferred Qualifications
At least 1-year experience in Human Services
Familiarity with Microsoft Office suite
Valid driver's license and willingness to drive agency van
