Job Description
Join HarmonyCares as a Nurse Practitioner and deliver personalized, quality healthcare in the home setting. You will conduct comprehensive health assessments, collaborate with a team of professionals, and make a significant impact on patients' lives by addressing their health needs effectively.
Key Responsibilities
Conduct comprehensive in-home health risk assessments
Takes history, examines, and determines diagnoses
Provides written documentation of patient visit per NCQA standards
Takes patient vital signs as necessary
Places case management referrals and communicates with PCP as necessary
Communicates with patients, caregivers, agency nurses, and other providers
Maintains patient confidentiality
Attends required meetings and participates in professional development activities
Required Qualifications
Active/unrestricted medical license
Active CPR Certification
Board eligible or board certified in family medicine
Outstanding EHR skills
Preferred Qualifications
Geriatric training/experience
Skill in teamwork and maintaining effective working relationships with patients, medical staff, and the public
