Job Description

Key Responsibilities
Participate in ongoing interdisciplinary assessment of the patient
Develop and evaluate the plan of care in partnership with the patient and caregiver(s)
Provide services ordered by the physician as indicated in the plan of care
Recognize and report changes in the patient’s condition to the RN
Counsel and educate the patient and family in meeting healthcare related needs
Complete timely and accurate clinical notes
Required Qualifications
Current license as a Licensed Practical Nurse (LPN) in the State of MN
At least 1200 hours of experience as an LPN/LVN or completion of the Office's preceptor program
Valid driver's license, reliable transportation, and proof of auto insurance
CPR Certification
Preferred Qualifications
2 years experience as an LPN
