The Case Associate at The Jewish Board will serve as a case planner responsible for ensuring client care through community integration and resource utilization. This role involves conducting home visits, providing case management, and facilitating access to services tailored to client needs.
Key Responsibilities
Function as a case planner and ensure client care
Develop action plans utilizing existing and new resources for clients
Conduct routine community contacts and evaluations
Run groups under supervision of a social worker
Perform individual/group counseling based on client needs
Identify and facilitate access to treatment or service providers
Conduct home visits to assess family and child safety
Provide case management and community outreach
Required Qualifications
BA Degree
Experience in a community or social service group
At least five years of experience as a Case Assistant in a community or social service group
Preferred Qualifications
Bilingual Haitian-Creole
