As a Care Coordinator, you will play a vital role in connecting individuals to essential health services, developing personalized care plans, and managing ongoing support. You'll work collaboratively with a diverse team to ensure members receive the care they need to thrive.
Key Responsibilities
Link adults and children with chronic behavioral health and medical conditions to necessary services
Develop Individualized Care Plans (ICP) with members and their families
Provide required care management services and track referrals
Conduct periodic assessments of members' health needs
Provide outreach services and respond to member inquiries
Complete all required documentation and respond to crises
Required Qualifications
Bachelor's degree in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation therapy, counseling, community mental health, child and family studies, sociology, or speech and hearing
Two years of experience in providing direct services or case management to mentally disabled, chronically ill, or homeless individuals
Ability to engage with clients and provide culturally competent services
Strong verbal and written communication skills
Preferred Qualifications
Experience working in interdisciplinary teams
Fluency in a second language such as Spanish, Russian, or Creole
