Job Description

Key Responsibilities
Identify ongoing needs and services of residents
Ensure proper follow-through and documentation for residents with a change in healthcare services and status
Assist Resident Care Director in conducting evaluations and data gathering
Conduct monthly wellness visits for all residents
Monitor and follow up with residents after incidents
Document all pertinent information in resident wellness files
Maintain communication with residents' families to ensure medical needs are met
Contact attending physician when necessary
Ensure weights and vital signs are obtained monthly
Maintain wellness files according to policies and regulations
Assist in maintaining medical supplies and emergency kits
Provide clinical support to community team members
Understand and follow infection control practices
Monitor medication and treatment profiles for residents
Participate in quality assurance and clinical meetings
Required Qualifications
Graduate of approved college/school of nursing
Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN)
Minimum of one year experience in home health, assisted living, hospital, or long-term care environment
Knowledge of nursing practices, techniques, and methods applied to health and wellness resident care
Good assessment skills
Ability to handle multiple priorities
Effective written and verbal communication skills
Organizational and time management skills
Good judgment, problem solving, and decision-making skills
Proficiency in computer skills, Microsoft Office, and Sunrise applications
Ability to work weekends, evenings, and flexible hours
