Nationwide Therapy Group is seeking a travel nurse RN Case Management for a travel nursing job in Sunnyvale, California.

- Admit patients into hospice care and perform comprehensive assessments of physical, psychosocial, environmental, safety, and developmental needs.
- Create individualized plans of care in collaboration with patients, families, and the multidisciplinary hospice team
- .Maintain accurate and timely clinical documentation reflecting assessments, interventions, outcomes, and changes in condition.
- Work closely with physicians, social workers, spiritual counselors, and other team members to ensure holistic, compassionate care.
- Conduct ongoing patient visits to monitor condition, manage symptoms, and provide emotional support.
- Participate in the program’s Quality Assessment and Performance Improvement (QAPI) initiative.
- Educate and support family members and caregivers about disease progression, hospice philosophy, and available resources.
Minimum 1 year of hospice nursing experience Willingness to travel across assigned service areas Strong communication, assessment, and organizational skills
