Job Description

The Case Planner at The Jewish Board plays a crucial role in providing preventive services and support to at-risk families, helping them develop skills to manage crises and ensure child safety. This position involves in-home assessments, counseling, and collaboration with community resources to enhance family well-being.

Key Responsibilities

  • Provide Child Welfare Preventive Services within the client's home and community
  • Assess and address safety and risk concerns related to children's health and well-being
  • Evaluate living conditions of clients through home-based contacts
  • Provide supportive case work counseling to children and family members
  • Identify and provide access to specific concrete services and community resources
  • Collaborate with external resources in the family's ecology
  • Conduct specialized groups for children, adolescents, and parents
  • Attend conferences at ACS offices and other community-based meetings
  • Complete weekly progress notes and enter data into PROMIS
  • Complete and update Family Assessment Service Plan Reports (FASP)
  • Participate in internal and external continuous quality improvement projects
  • Monitor children regarding their medical needs
  • Participate in Family Team Conferences (FTCs)
  • Perform outreach to communities and establish linkages with other groups

Required Qualifications

  • BA or Masters level degree in Psychology, Social Work, Counseling or a related subject
  • Basic social work knowledge and understanding of stages of change
  • Ability to provide in-home child welfare services
  • Ability to establish rapport and therapeutic relationship with families
  • Knowledge of Microsoft Office

Preferred Qualifications

  • Understanding of the use of family therapy principles to provide services
  • Understanding of early childhood development and related theories and models of practice
  • Strong Assessment skills to evaluate dynamics and interactions within the family
  • Knowledge and experience with the Connections system
