Join MJHS as a Hospice Field Case Manager, where you'll support patients and families in hospice and palliative care. In this role, you'll conduct assessments, develop care plans, and collaborate with healthcare teams, all while making a meaningful impact in patients' lives.
Key Responsibilities
Visit patients to perform physical and psychosocial assessments
Electronically document visit occurrences
Develop and implement nursing plans of care
Contact patient's physician for care plan changes
Supervise, train, and evaluate home health aides and homemakers
Required Qualifications
Associates/Diploma from an accredited school of Nursing
New York State RN licensure
Valid NYS driver's license
Effective communication, listening and teaching skills
Ability to input clinical data proficiently
Preferred Qualifications
BSN
At least two years experience in medical-surgical, community health, and/or oncology nursing
Proficiency with Word and Excel
Experience working within an interdisciplinary team environment
