Join Molina Healthcare as an IRIS Consultant and make a meaningful impact in the lives of individuals with long-term health care needs. This remote role allows you to build relationships and help participants navigate their care options effectively.
Key Responsibilities
Build relationships with IRIS participants and assist them in navigating the Wisconsin IRIS program
Identify long-term care goals and develop customized IRIS plans
Document all orientation and planning activities within the IRIS data system
Research community resources and assist participants with provider billing
Attend in-person monthly team meetings and participate in weekly check-ins
Required Qualifications
Bachelor’s degree in social work, psychology, human services, counseling, nursing, special education, or a closely related field (or four years of commensurate experience if no degree)
1+ year of direct experience related to the delivery of social services to individuals with intellectual or physical disabilities and older adults
Valid driver's license with good driving record
Preferred Qualifications
Knowledge of Long-Term Care programs
Familiarity with principles of self-determination
Excellent problem-solving and critical thinking skills
Strong basic math skills
Excellent time management and prioritization skills
Ability to develop and maintain professional relationships
Resourceful with knowledge of community resources
Proficient in technology including Outlook, Skype, Teams, PowerPoint, Excel, Word, online portals, and databases
