Key Responsibilities
Develops, assesses, and facilitates complex care management activities for mental and behavioral health needs
Evaluates member needs via phone or in-home visits and facilitates care plans
Performs telephonic, digital, home, and/or other site visits outreach
Coordinates between the member, family, community resources, and care provider team
Monitors care plans and member status for changes in treatment
Facilitates care coordination with providers and specialists
Documents member information and care management activities
Educates members and families on healthcare options and benefits
Provides feedback for opportunities to improve care delivery
Required Qualifications
Must be licensed and reside in the state of Florida
Pediatric Experience Required
Licensed Behavioral Health Professional or RN
Master's degree in Behavioral Health or Social Work or Degree from an Accredited School of Nursing
2 – 4 years of related experience
