Key Responsibilities
Conduct in-home assessments including medical history and physical exams
Perform evidence-based practice screenings
Identify diagnoses for care management
Formulate a list of medical conditions
Communicate findings to PCP
Identify urgent situations and intervene
Educate members on disease processes and compliance
Required Qualifications
Active NP license or ability to obtain by start date
National certification in Family, Geriatrics, Adult, or Adult-Gerontology
Ability to complete physical requirements of the job
Driver's License and access to reliable transportation
Preferred Qualifications
1+ years of clinical experience
Experience in Gerontology, Cardiology, Internal medicine, or Endocrinology
Home Health care or home visit experience
